Red flags — walk away if you see these
  • No psychological assessment.
  • BMI requirements waived.
  • Surgery offered within 7 days of first enquiry.
  • No long-term aftercare plan.
  • Pre-op diet not required.
  • Sleeve performed in a day clinic without ICU.

Eligibility and assessment

  • Independent medical assessment confirming you are a suitable candidate.
    NICE criteria: BMI ≥40, or ≥35 with weight-related conditions.
  • Psychological assessment before surgery.
    Reputable bariatric programmes require this. If skipped, that is a warning.
  • Pre-op liver-shrinking diet 2 weeks before surgery.
    Reduces surgical risk significantly.

Surgery and surgeon

  • Surgeon completes ≥100 bariatric cases per year.
    Volume is the single strongest predictor of safety.
  • Surgery in a JCI- or NABH-accredited hospital with ICU.
    Bariatric leaks are emergencies. ICU is not optional.
  • Procedure choice driven by clinical assessment, not your preference.
    Sleeve vs bypass vs balloon: each has different indications.
  • Leak test performed intra-operatively.
    This is standard practice. Confirm in writing.

Aftercare — the part that matters most

  • Lifelong vitamin and mineral supplementation plan.
    B12, iron, calcium, vitamin D, multivitamin. Confirm in writing.
  • Annual blood-test schedule with a UK GP.
    Your clinic should provide a template for your GP.
  • Dietitian follow-up at 1, 3, 6 and 12 months.
    Video calls are acceptable if in-person isn't possible.
  • Psychological support post-surgery.
    Eating disorders can shift form after bariatric surgery.
  • Confirm what happens if complications arise back in the UK.
    NHS will treat emergencies, but elective revision is rarely funded.
